Overview:

Appear before a Passort Acceptance Agent at a U.S. Post Office or Clerk of the Court.  They will verify your documents, administer an oath, and you will be asked to sign your application.  Your documents will be placed in an envelope and sealed with their official stamp. DO NOT leave your documents with the clerk, you should be handed a sealed envelope to take with you. Ship that sealed envelope to our office via FedEx Priority.


Instructions:

    • This application MUST BE COMPLETED ONLINE , please click the "Create Form" button at the end of the screen.  By doing so, it will create a PDF version of your application.
    • Please print two (2) sets/copies of this application.  (Be sure to review your application. Applications with handwritten corrections will be rejected by the U.S. Department of State.
    • Do not sign the application until instructed to do so by a passport acceptance agent or clerk of the court.
    • One copy will be endorsed by the acceptance agent/clerk and sealed in an envelope.  The second copy does not need to be endorsed, but should be paper-clipped to the outside of the sealed envelope.
  1.  Submit Two (2) Letters of Authorization
    • Please complete and sign TWO Letter of Authorization.  Original signatures are required.  No photocopies.
    • One copy will be placed inside the sealed envelope by the passport acceptance agent/clerk, and the other copy should be paper-clipped to the outside of that envelope.
  2.  Submit Proof of U.S. Citizenship
    • Certified Birth Certificate with raised/embossed state seal (not a hospital record); or
    • Certificate of Naturalization or Citizenship; or
    • Consular Report of Brith Abroad; or
    • A previous U.S. passport in good condition
  3.  Submit Two (2) Recent Passport Photos (2"X2")
    • Photos must be within 6 months and must be professionally made.  Passport photos are available at Walgreen's, CVS, and FedEx Office.  Computer, snapshot, and homemade photos are not acceptable, and will be rejected by the U.S. Department of State.
  4.  Submit Proof of Travel
    • Submit an itinerary or a business letter from your company showing your travel plans. For expedited passport service travel date on itinerary must be within 14 days of submitting your application or 21 days if a visa is required.
  5.  Submit Passport Book Fee
    • Please do not write any checks until instructed to do so by the passport acceptance agent/clerk.
    • A check or money order should be made payable to the "U.S. Department of State"
    • $170 - Adults (age 16 and over)
    • $140 - Minors (age 15 and under)
  6.  Submit Execution Fee (Acceptance Facility)
    • Please do not write any checks until instructed to do so by the passport acceptance agent/clerk.
    • A check or money order should be made payable to the "Postmaster" or "Clerk of the Court"
    • $25 - Execution Fee
  7.  If Applicable, See Special Requirements for Minors (age 17 and under)
  8.  Show Valid Personal Identification
    • Present valid photo ID or Driver's License.

Step 1

Process your passport: 

Take items 1-9 to the Post Office or Clerk of the Court

  • At the Post Office or Clerk of the Court, you will take an oath and be asked to sign one copy of the application.
  • The acceptance agent/clerk will place items 1-7 (or 1-8 when applicable) in an envelope, seal it, and affix their official stamp on the envelope. Do not open the envelope once sealed.
  • When processing multiple applications:
    • IMPORTANT - EACH INDIVIDUAL MUST HAVE THEIR OWN SEPARATE ENVELOPE.  Be sure the agent/clerk does NOT seal multiple applications in one envelope. The Department of State will not accept them.
    • VERY IMPORTANT - you MUST contact one of our offices to make a reservation for the 2-3 Day Expedited service. Space is very limited. Fees are non refundable once reservation is made.

  • Ship the following documents to the correct office handling your request using FedEx Priority Overnight
    • Official sealed envelope
    • Second copy of completed DS-11 application and a second copy of the DS-64 form
    • Second letter of authorization
    • ALL APPLICATIONS MUST BE RECEIVED IN OUR OFFICE BY 9:00 A.M., PREFERABLY ONE BUSINESS DAY BEFORE YOUR SCHEDULED RESERVATION APPOINTMENT.
    • Do not ship any packages for Saturday delivery

Our company cannot guarantee the issuance or processing time of any visa or passport.

The issuance and processing time is determined by the respective Embassy or Consulate

for visas, and the U.S. Department of State for passports.

All fees are non refundable once the application has been submitted to the respective Embassy,

Consulate, or the U.S. Dept. of State, even in cases of denial.

The majority of delays are caused by incomplete applications or missing requirements.

We suggest not purchasing your airline ticket until your visa and/or passport have been secured

and returned to you. We cannot be held liable for airline tickets, hotels, car rentals or any other

 travel related expenses incurred.
