Tourist Visa Requirement Checklist

Step 1

 Send the following items to our Miami office via FedEx Priority

1) Submit your original, signed passport valid for at least 6 months beyond stay in Bolivia, with at least one blank visa page available for the visa stamp. Amendment pages in the back of the passport cannot be used for visas.
  2) Submit one Bolivian visa application form per applicant properly completed and signed. Signature must be original. Please be sure all questions are answered. 
3) Submit three recent 2 x 2 passport type photograph in color, front view and with a plain/white background. No glasses allowed. Only professional photos will be accepted. They can be taken in CVS, Walgreens or FedEx Office.
 4) Submit proof of travel, a computer generated flight itinerary from the airline or a travel agency or copy of airline tickets. 
 5) Submit a copy of confirmed hotel and/or cruise reservation(s.) If staying in various hotels, please provide copies of each reservation. A letter from inviting family member or friend is required if they are providing accommodations to the applicant.
6) Submit your International Yellow Fever Vaccination certificate.
7) Submit proof of financial funds to support your stay in Bolivia. The Consulate will accept a copy of your recent bank statement or a copy of the front of your credit card.

7) Submit an invitation letter from the Bolivian company you will be visiting. The letter should contain your name and the dates of the trip.

8) Submit a business letter from your company, the letter should contain your name with the trip information, include the Bolivian company name and contact information.

Consulate, or the U.S. Dept. of State, even in cases of denial.

The majority of delays are caused by incomplete applications or missing requirements.
