Tourist Visa Requirement Checklist

Step 1

Send the following items to our Houston office via FedEx Priority

1) Submit your original Passport valid at least 6 months beyond stay, and containing at least one blank visa page, any amendment/endorsement pages in the back cannot be used for visas.

2) Submit (2) Malaysia Visa Application forms completely filled out and signed. Two original forms and one copy.

3) Submit (2) passport size photo, 2" x 2", white or light background, glued or stapled to the application form. Photos must be professionally taken. No glasses are allowed. Photos from Walgreens, CVS, or FedEx Office are recommended. Any domestic, family photos, snapshot, homemade, photocopies, and computer photos are not accepted by the Consulate.  

4) Submit proof of departure, an itinerary showing your flights in and out of Malaysia.

5) Submit a copy of the applicant's hotel reservation. If staying with a friend in Malaysia you will need an invitation letter along with a copy of the inviting person's passport.

6) Submit an original letter from your employer. The letter should state that the applicant is an employee of the company and leave was approved. Also state the intended date of return to the company. Retirees may submit proof of retirement fund. Students should submit a letter from the school  confirming enrollment status.

7) Submit proof of financial funds. A recent bank statement showing sufficient funds for the trip.

8) Minors under age 18 are required to submit a notarized copy of birth certificate. For a minor who is not traveling with both parents, please attach a notarized letter of consent signed by the non-accompanying parent.

9) Non U.S. Passports; The Embassy of Malaysia requires a notarized copy of your U.S. Resident Card. If you are not a U.S. Resident then you must have a valid U.S. visa with at least 6 months validity after leaving Indonesia and an original valid I94 with at least 6 months validity after leaving Indonesia.  

10) RUSH SERVICE: Applicants requesting Rush issuance (Premier Gold) or (2 weeks) should send the package to us via overnight Priority delivery. Please do not use First Overnight or Standard.

Business Visa Requirement Checklist

6) Submit a Business letter from U.S. based employer or sponsoring company, on company letterhead, introducing the applicant, indicating the applicant's position in the company, and detailing the purpose of visit to China. The business letter must also indicate who will be financially responsible for the applicant, provide detailed contact information in China, and specify the number of entries (single, double or multiple) being requested. Faxed or emailed copies are accepted.

7) Submit a notarized Letter Of Invitation  from the host company or inviting organization in Malaysia. The invitation letter should contain the applicants name, purpose of the trip, and a financial guarantee.

8) Non U.S. Passports; The Embassy of Malaysia requires a notarized copy of your U.S. Resident Card. If you are not a U.S. Resident then you must have a valid U.S. visa with at least 6 months validity after leaving Indonesia and an original valid I94 with at least 6 months validity after leaving Indonesia.
