The Woodlands:

Please follow all steps outlined below so we can process your passport application. Once all completed, you’ll meet with Cynthia at our Woodlands office. She will take your sealed envelope from you and we will submit it to the Department of State for processing. If you prefer to complete all the below in our office (except acceptance facility step 7), please call us for an appointment. Please note all appointments take approximately 30 minutes per person, and the courthouse step is still required. Please allow enough time in your day to finish your complete application process.

Adult/Minor New Passport Requirements:

Step 1 - Click here to complete the Service Order Form.
This link will take you our Payment and Processing Speed Request form. Here you will be able to choose the processing speed that best meets your traveling needs. There is no need to print, simply click the green “save” button when you are done. If completing the form via a mobile device, the save button will be blue. This is an electronic form and we will receive it instantly. A confirmation email will be sent to you confirming your assigned SOF number.

Step 2 - Create and Print two (2) DS-11 & DS-64 Passport Applications
Please leave the travel section blank on the DS-11. Once finished please print two sets of your form, DO NOT SIGN YOUR APPLICATION. Handwritten applications are NOT ACCEPTABLE.

Step 3 – Prepare Your Government Fee Payment.
For Adults: The Department of State REQUIRES a check or money order for their fee of $170 for the new passport book only (For a passport book and card it will be $200, and card only is $90). Your check or money order must be payable to the “DEPARTMENT OF STATE”. Do not abbreviate Department. Your check memo must state the applicants “FULL LEGAL name and Date Of Birth”.
For MinorsThe Department of State REQUIRES a check or money order for their fee of $140 for the new passport book only (For a passport book and card it will be $155, and card only is $75). Your check or money order must be payable to the “DEPARTMENT OF STATE”. Do not abbreviate Department. Your check memo must state the applicants “FULL LEGAL name and Date Of Birth”.

Step 4 – Print and Sign two (2) copies of our Letter of Authorization (only needed for expedited services).
To receive your L.O.A. please send an email to tshea@passportworld.com or cynthia.shea@passportworld.com with the full legal name of the applicant (first, middle, and last/suffix) and their phone number. Once received, print and sign. Original signatures are required, no photocopies are accepted.

Step 5 - Provide Two (2) Recent passport photos and Proof of U.S. Citizenship. 
Photos: Must measure 2"x 2", be within 6 months and must be professionally made. Computer, snapshot, and homemade photos are not acceptable, and will be rejected by the U.S. Department of State. Photo Ready Requirements: No uniforms, no glasses, neutral look, no head gear or hair accessories. Please don’t use the same photos as in your current passport book. Remember our office can take your pictures during your appointment if you choose to use the full service option.
Proof of Citizenship: Original Certificate of Birth in the U.S., Original Certificate of U.S. Citizenship, a previous old U.S. Passport, or Certificate of Birth Abroad from a US Embassy, whichever applies to you. Photocopies are not acceptable. (Make a photocopy for our records).
For children: Original Certificate of Birth in the U.S. (will show parental relationship to the child) or Original U.S. Naturalization Certificate or Original Certificate of Birth Abroad, whichever of the three options apply to you. If the child has had a passport issued to them in the past, they need to submit it as well. If it is lost, please note your passport application will print out as a DS-11 and DS-64. (Make a photocopy for our records).

Step 6 Provide Proof of Travel (only for expediting services).
Submit an itinerary showing your travel plans or a letter explaining travel plans. Please note your departure date must be within 14 days of the application date. Email us if you do not have one.

Step 7 – Complete your OATH:
Take all your documents completed from Steps 2-6 and go to the Montgomery County District Clerk office to execute one (1) set of original copies. This acceptance facility is only open Monday through Friday from 8:30 am - 4:00 pm. They are an appointment only facility. Please call us ahead at least 24 hrs before your desired date so we can assign you an appointment time. You need a Government Issued ID to complete this step! For children: Both parents must be present with ID’s. If ONE parent cannot be present, call us so we can tell you your options. They will collect $35 for executing your application in only cash. If you are applying for routine service they will keep your documents and send it to the National Passport Center and you will receive your new passport via USPS Priority Mail. If you are doing Expedited Services, after you have executed your application at the Courthouse they will give you a sealed envelope. Your extra set of copies must be paperclipped to the outside of your sealed envelope. If we do not receive the exact same copies as your sealed envelope we cannot process your application. The Montgomery County District Clerk office is located at 301 N Thompson Street, 2nd floor, Suite 203, Conroe, TX, 77301.

Step 8 – Drop off your documents.
Your sealed envelope and copies must be dropped off at our Woodlands Office for processing (only for expedited services, routine services do not get a sealed envelope from Step 7). Our office is located at 2002 Timberloch Place, Suite 200, The Woodlands, TX, 77380. Our business hours are Monday through Friday from 9:30am-5:00pm (closed 12-1pm).

Frisco:

Please follow all steps outlined below so we can process your passport application. You’ll meet with Ashley or Sarah in our Miami office. She will take all the below documents from you and send them to our Houston headquarters for processing. You can also overnight your documents to our office directly at: PassportWorld, LLC., 2002 Timberloch Place, Suite 200, The Woodlands, TX, 77380.

Step 7 – Complete your OATH.
Make two copies of Steps 2-6 and go to the Frisco Denton County Clerk office to execute one (1) set of original copies. This acceptance facility is only open Monday through Friday from 8 am - 4 pm. They are a walk in facility and do not require appointment. They will collect $35 for executing your application in only cash. After you have executed your application at the Courthouse they will give you a sealed envelope. Your extra set of copies must be paper-clipped to the outside of your sealed envelope. If we do not receive the exact same copies as your sealed envelope we cannot process your application. The Denton County Clerk office is located at 5533 FM 423, Frisco, 75036.

Step 8 – Drop off your documents.
Your sealed envelope and copies must be dropped off at our Frisco Office or you can FedEx your documents to our Woodlands, TX Office for processing (our preferred option). Our Frisco office is not a full service location, but we are always a phone call away to assist you. Our Frisco office is located at 7460 Warren Parkway, Suite 100, Frisco, TX 75034 and is open M-F from 9 am-5 pm. If you are sending your documents directly to our main office via FedEx, Our office address is: PassportWorld, LLC., 2002 Timberloch Place, Suite 200, The Woodlands, TX, 77380. Our business hours are Monday through Friday from 9:30 am - 12:00 pm (closed for lunch) 1:00 pm - 5:00 pm. Please do not FedEx your documents for early am delivery (we open at 9:30 am) or for Saturday delivery (we are closed). USPS Mail deliveries are placed into a box that we check once a week, so we advise you not to deliver through USPS. Please only use FEDEX.

Coral Gables:
